NO Walk-in Medication refills. You must call 5-7 business days prior to your refill pick up date to allow us time to review your chart and prepare your prescriptions.

If you do not bring a valid insurance card and valid ID to your appointment, you will not be seen.
There will be a $25 fee any time you do not give 24hr advance notice if you cancel your appointment. You cannot be seen, nor pick up scripts until that NCNS fee is paid.
If you are receiving controlled substances, you must be seen every two months.
If you are requesting a controlled substance medication change, you MUST return the unused portion of your current medications.
You will not receive a new script unless you return your previous prescription, if it was not filled, or your remaining medication. Medications should not be flushed down the toilet or thrown away.
If you are currently our patient, and you provided the office your email address when you checked in, you would have obtained an email from us inviting you to the patient portal. Please take the time to set up your patient portal account. The patient portal is an easy, convenient, and secure way for you to communicate with the office. You may access the patient portal by clicking the blue Athena Portal link above. The portal allows you to:
- access to personal health information
- message the staff directly
- submit medication refill requests
- view account balances, statements, and pay bills online
- schedule or request appointments
